Project Description

The PEPFAR Community Grants Program is funded through the U.S. President’s Emergency Plan for Aids Relief (PEPFAR). One of the goals of PEPFAR in Mozambique is to support efforts to achieve HIV epidemic control through implementation of evidence-based interventions to drive progress in HIV prevention, care and treatment and ultimately, save lives

 

Through the PEPFAR Coordination Office, the U.S. Embassy in Maputo administers the

Community Grants Program (formerly Small Grants) and is seeking proposals from registered

community-based organizations whose mission and activities focuses on HIV programming.

 

Objectives

 

As PEPFAR continues to confront challenges with retaining HIV+ individuals in care and treatment programs, our collaboration with communities and patients is urgent and critical. Collaborating with community members that will identify barriers and enablers to accessing and utilizing HIV services will be key in improving retention in Mozambique, as will ensuring stigma and discrimination surrounding HIV is addressed. Organizations will be expected to carry out patient surveys and gather information on the patient experience at health facilities. This information will then be used to facilitate dialogue and advocacy for improvement. Training and technical assistance will be provided to organizations selected for this award.

 

This NOFO seeks eligible organizations to train, support, equip and compensate PLHIV to carry out routine monitoring of the quality of HIV treatment services received by patients at health facilities. We are looking for organizations that can help communities directly impacted by HIV to facilitate dialogue, based on patient experiences, that will help drive changes and improve HIV services in Mozambique.

 

This mechanism will help PEPFAR programs, Public Health facilities and Local Government

Institutions to gain insights on persistent problems impacting the quality of care of HIV and identify solutions or enablers that will ensure that beneficiaries receive quality services and will encourage them to retain in care and adhere to their HIV treatment.

1.    FEDERAL AWARD APPLICATION INFORMATION

For all application documents, please ensure that all documents are in English and all costs are in U.S dollars.

 

1.1     Applicants should submit ONE project proposal that does not exceed TEN pages

1.2     All documents are 1.5 spacing, 12-point Times New Roman font

1.3     All pages are numbered including budgets and attachments

1.4     Each proposal MUST include also a one-page summary that includes an Executive summary, outlining project goals, objectives, proposed cost and activities.

 

2.    APPLICATION REQUIREMENTS

Complete application MUST include the following:

 

2.1     Completed and signed SF-424 and SF-424B forms

2.2     Cover Page (not exceeding ONE page, word document) including a table with organization name, project title, target districts/location, name and contact information of the organization’s primary point of contact

2.3     Table of Contents (not exceeding ONE page) listing all documents and attachments with numbers

2.4     Proposal Narrative (not exceeding FIVE pages). Please note the ten pages DOES NOT include the Cover Page, Executive Summary, Table of Contents, Attachments, Detailed budget, Budget Narrative, Audit or NICRA

2.5     Budget amount of funding request (excel workbook), costs must be in U.S dollar not exceeding $30,000

2.6     Expected measurable results (not exceeding ONE page)

2.7     Budget Narrative (word document) that includes substantive explanations and justifications for each line item in the detailed budget (not exceeding ONE page)

2.8     Your organization’s recent audit or 1-2 examples of recent positive performance

2.9     1-page CV or resume of key personnel who are proposed for the program with their role description

2.10  Monitoring and evaluation narrative plan (not exceeding TWO pages, word document)

2.11  All organizations must include their DUNS number on their proposal.

Applications that do not include the elements listed above will not be considered.

 

3.    REQUIRED REGISTRATIONS

 

3.1  All applicants must obtain the below registrations (all are free of charge). Below are links to each registration which must be completed.

 

3.2  All organizations must obtain a unique entity identifier from Dun & Bradstreet (DUNS number) and note their DUNS number on their proposal.

4.    ELIGIBILITY AND FUNDING RESTRICTIONS

The PEPFAR Community Grants Office welcomes applications from registered non-profit Mozambican organizations that are led by People Living With (Infected) or Affected by HIV focusing on Key Population groups (female sex workers, men who have sex with men, people who inject drugs, and prisoners), youth (adolescent boys and adolescent girls), young men, and HIV+ individuals with disabilities.  

 

●       All applicants must be a registered Non-Profit Organizations (NPOs) and have been in operation for at least one year to be eligible for funding.

●       Organizations must be able to demonstrate that they have adequate internal controls and financial monitoring procedures in place.

●       Organizations should demonstrate strong ties and support in the local community and with local governmental authorities.

 

Each project accepted for funding will submit financial and narrative progress reports on a routine basis. Grantees will be instructed on the inputs required for financial and programmatic progress reports by the PEPFAR Coordination Office.

 

5.    DEPARTMENT OF STATE FUNDING RESTRICTIONS

Per Department of State policy, funding cannot be used for the following purposes:

●       Construction of buildings;

●       Salary, rent, utilities or office furniture related to the organization applying for this funding;

●       Land purchase, security guards, motor vehicles (including motorcycles, but excluding bicycles), car insurance, motor vehicle repairs or upkeep;

●       Outside audits;

●       Purchase of alcoholic beverages; and

●       Activities contrary to the Protecting Life in Global Health Assistance Policy.

 

6.    FUNDING ALLOCATION

Funding will be limited to a maximum of $30,0000 per implementation period for each project (minimum period of implementation is for six month). Funding is expected to be allocated in the districts listed below; applicants may only send a proposal focused on one of the chosen geographic area. Proposals that do not follow the guidelines above, categories described, and districts indicated will not be considered.

 

Geographic Priorization Area:

Maputo Province (Matola, Boane, Magude, Manhiça, Marracuene, Matutuine, Moamba and Namaacha);

Gaza (Bilene, Chibuto, Chockwe, Chonguene, Guijá, Limpopo, Mabalane, Mandlakaze, Mapai, Massingir and Xai-Xai); and

Inhambane (Funhalouro, Govuro, Homoine, Inhambane, Inharrime, Inhassoro, Jangamo, Mabote, Massinga, Maxixe, Morrumbene, Panda, Vilankulo and Zavala).
